What is a health risk assessment?
This assessment is an
excellent way for you to learn about your overall health status. The health
risk assessment does not replace seeing your own doctor and in fact you will
need to have some information from your doctor’s office to complete the
assessment in order to get the best picture of your overall health. The East
Central District Health Department believes that knowing what your health
risk is can be the first step toward a healthier way of life. By taking the
health assessment you will learn about your overall wellness score for your
age. This score takes into account your age, weight, exercise habits,
nutrition, how you handle stress, family and personnel history. It will also
give you detailed information on how to reduce your personnel risks for
cancer and heart disease two of the leading causes of death. In addition you
can use the information contained in the assessment to set some personal
goals. You can retake the assessment every six months to find out how your
lifestyle changes have affected your overall health status and your overall
wellness score.
What do I need in order to determine my health risk?
The assessment will take about 10-15 minutes, you might want access to a
printer so that you can print your health assessment results right away. To
get the most out of this on-line assessment tool you will want to contact
your health care provider for some basis information. You should know your
height, weight, usual or last blood pressure reading, your last total
cholesterol reading, your HDL cholesterol reading and your blood glucose
level. If you do not have this information you can still take the free
assessment, the assessment however, uses these health indicators to
calculate your specific risk for your age, without them it will not give you
as specific of information about your health status. If you have not had
these assessments recently or if you have never had these basic health
assessments done you can contact your healthcare provider for assistance. If
you do not have a health care provider you can receive these assessments at
East Central District Health Department for a fee based on your income. Call
the ECDHD and ask for a TRALE assessment appointment.
How do I take my personal health assessment?
When you click on the link below you will find yourself at the first
login page. There will be two separate login pages. The first page asks for
a login ID and a password. To make this as easy as possible we have included
the proper login which is public and the first password which is health.
Once you have typed in these words than click on login. This will take you
to a second page where you will be asked to create a unique username and
password. This is important so that you can log back into the system to
access your personal health assessment survey in the future and so that you
can print out your report.
You might want to choose an easy-to-remember username. Many people simply
choose their first initial and their last name or their first name and last
initial. For example Bobby Smith might choose:
You may choose anything you like that you can remember as long as it has
six characters (letters and/or numbers)
Once you have put in your username that is unique for you, you must
choose a password that is unique to you. Be sure to write your password
somewhere so that you can remember it to get back into the assessment in the

If you are taking the assessment as part of an ECDHD health fair or if
you are taking the personnel health assessment on the recommendation from
your doctor they will give you a site code. This site code will not identify
you, however, it does help to identify the health fair or doctors office
where the assessment was recommended. If no site code was provided to you
simply enter the code 000.
How can I have this assessment given at a health fair, my workplace or to
the individuals who visit my office?
The East Central District Health Department pays for the service that
allows for this individual personnel health assessment. The health
department would be delighted to bring this survey to your health fair or
agency. In addition it is our desire that hospitals, medical providers and
occupational health programs within your county will be able to use this
survey in their offices to help all the residents in our four county area
know what their overall wellness score is and know how to make lifestyle
changes to improve their wellness score. If you are interested in having
